Lost Cheque

If you are working or have worked on a production with EP Canada as your production’s payroll service provider and your cheque has been lost or stolen, you may request reissue of your cheque.


If you are currently working on the production where your cheque has become lost or stolen, please contact the Production Accountant for additional direction and information for processing this request.
 
If you are NOT currently working on the production where your cheque had become lost or stolen, please follow the steps below.
 
In order to complete the request to Reissue a Lost Cheque:
 
1.Complete the Stop Payment Authorization Form
·Download the form from our website www.epcanada.com under the "Download Centre” Payroll Forms section, Other Forms and complete it as necessary
·Select your option on the form
·Amount deducted from reissued cheque by Employee Consent
·Please print your name and sign at the bottom of the form as the Payee
·Please let us know if your address has changed recently

 
2.Send EP Canada the Stop Payment Authorization Form

  1. EP Canada processes your request
    • The turnaround for a Lost Cheque is up to 5 business days

       
  2. Your cheque is reissued
    • EP Canada will mail your cheque to the address you specified on your Deal Memo/Start Slip, unless otherwise specified with your request


Stolen Cheque

In order to complete the request to Reissue a Stolen Cheque:
 
1.    Complete the Stop Payment Authorization Form
·       Download the form from our website  www.epcanada.com under the "Download Centre” Payroll Forms section, Other Forms and complete it as necessary
·       Select your option on the form
·   Amount deducted from reissued cheque by Employee Consent
·   Please print your name and sign at the bottom of the form as the Payee
·   Please let us know if your address has changed recently
 
2.    Send EP Canada the Stop Payment Authorization Form

    • The turnaround for a Stolen Cheque is up to 5 business days

       

  2. If we determine that the cheque has been cashed, we will contact you to discuss. If the cheque is determined as Stolen then we will  require an Affidavit Declaration form that you may download from our website  www.epcanada.com under the "Download Centre” Payroll Forms section, Other Forms which will include:
    • Amount of cheque and currency of issue, for example (Cdn $100.67)]
    • Account number to which the cheque was issued from
    • Cheque date
    • Please have a witness sign the form
    • Once we have all the required completed paperwork, the turnaround for this process can take up to 30 business days

  3. Your cheque is reissued
    • EP Canada will mail your cheque to the address you specified on your Deal Memo/Start Slip, unless otherwise specified with your request
